Job DescriptionQUALIFICATIONS:Registered Nurse from an
accredited program currently licensed and in good standing with the
State Board of Nursing, with one year of administrative or
supervisory experience. Must have strong clinical skills with
hospice and/or home health experience. Must have excellent verbal
and written communication skills, as well as good community
relations. Must have the ability to motivate and work with fellow
employees and volunteers. Must be effective in working with
different levels of health care providers, and able to express the
hospice concept to families, patients, physicians, community,
volunteers, staff and other professionals. This position requires
planning, supervision and administration. Hospice and Palliative
Care certification preferred, or the understanding that this must
be obtained within 6 months of being eligible to sit for the
certification test.RESPONSIBILITIES:It is the responsibility of the
Director of Clinical Services to provide leadership and effective
management for the Interdisciplinary Team. The Director of Clinical
Services is accountable and responsible for coordination and
ensuring the highest level of hospice care is provided to patients
and families. The DCS acts as a liaison between the IDT and the
Executive Director.The Director of Clinical Services works under
the direction of the Executive Director and in conjunction with the
Interdisciplinary team to:
- Monitor to ensure coordination and follow through is complete
for all referrals to the hospice program
- Assist admission team in coordination and assignment of all
newly admitted hospice patients to the appropriate team and/or
discipline, i.e., admission nurse, care manager or other RN
- Work collaboratively with the medical records department to
ensure all admission documentation is completed within 12-hour
period and dispersed among team members
- Assign a Nurse Care Manager to implement the Plan of Care and
oversee the delivery of care to the patient/family
- Assist the medical records department in monitoring the
documentation of all written material related to patient care
- Act as triage nurse for care managers requiring assistance with
symptom management
- Assist with change in level of care for patients including,
locating a facility for placement, ensuring a physician for follow
up at the facility, notification of primary care physician and
medical director, transport, provision of medications and DME
- Supervise and evaluate the scheduling of C.N.A.'s, contract
employees, nurse care managers, visit nurses, continuous care
staff, on call nurses.
- Ensure communication of all patient admissions and changes in
Plan of Care is appropriately relayed to each member of the
Interdisciplinary Team and Primary Care Physician
- Ensure policies and procedures concerning patient care are
followed
- Ensure and participate in annual evaluation of clinical
staff
- Recruit, hire and train all clinical staff
- Provide orientation of symptom management and medication
utilization to all nurses and physicians.
- Provide and/or assist in ongoing training of clinical
staff
- Participate in the QAPI program
- Implements practices that ensures Family Satisfaction measures
exceed national benchmarks
- Ensure that the highest ethical standards are practiced
throughout the organization
- Monitor the medication profiles for each patient on service to
identify formulary and non-formulary utilization.
- Identify ways to reduce non-formulary medication
utilization
- Participate with medical directors to ensure appropriate and
timely evaluation and treatment of all hospice patients
- Participate in communication with patients and families with
concerns/frustrations related to services and care.
- Provide continuous support to all clinical staff
- Monitor for and ensure appropriate utilization of "Resource
Book" and "Formulary" medications in the care of hospice
patients
- Participates in all IDT meetings in which admissions,
certifications, re-certifications and medical management of the
patients are discussed.
- Coordinate and present or arrange for in-services specific for
clinical staff throughout year
- Meet at least monthly with skilled nursing staff to discuss job
related issues and provide support
- Meet at least monthly with C.N.A.'s to discuss job related
issues and provide support
- Participate in and/or assist with formal evaluation annually
for clinical staff
- Provide annual evaluation for all clinical staff
- Perform joint visits with clinical nursing staff and certified
nursing assistants on a regular basis and minimally annually
- Ensure required supervisory visits are done on a routine basis
for all LPN"s and CNA's
- Participating in routine visits and admissions of patients in
times of high census or low staff availability
- Participate with the management of admissions and patient care
in times of high census or low staff availability
- Assist in staffing or reassigning visits to other team members
when staff member is ill or on vacation
- Participate with program development, community networking and
community/medical staff education to achieve census building
- Assist with the volunteer and bereavement programs
- Participate in budgetary decisions related to cost
effectiveness, and appropriate decision making by clinical staff,
i.e. utilization of pharmaceuticals, DME, and patient care
supplies, salary, overtime, etc.
- In the absence of the Administrator, perform administrative
duties as necessary
- Ensure adequate supervision when utilizing time off
- Provide input into Policy and Procedure development, i.e.
revisions, program development and new markets.
- Assist with timely and appropriate billing.
- Provides community education.PROFESSIONALISM:
- Work as a team member at all times and uphold the Hospice Code
of Ethics.
- Ensures a patient centered culture through CARE (Compassion,
Advocate, Respect, and Educate).
- Work as a team member to fulfill all aspects of Our Gold
Standards on a daily basis.
- Providing a positive image of the Hospice at all times.
- Balances team and individual responsibilities.
- Promote professional standards of behavior and ensure all staff
members are aware of their personal accountability and
responsibilities.
- Adheres to all HIPPA, HITECH, EPHI and company policies and
procedures regarding patient protected information.ORIENTATION AND
TRAINING:Hospice will provide orientation and training, as
necessary, to assist the Director of Clinical Services in
performing the responsibilities listed above. Trainings may be
in-house, or outside seminars. Evidence of orientation and training
will become part of the personnel file.JOB REQUIREMENTS:Job
requires the ability to:
- Access patients' residence and all rooms therein
- Reliable transportation
- Communicate effectively with patient/families both verbally and
written
- Communicate the mission and vision of the hospiceWORK
CONDITIONS:May require sitting or standing for prolonged periods of
time. Some driving/travel required. May require occasional lifting,
pushing or pulling up to 50 lbs.ON CALL:The Director of Clinical
Services will be on call, as necessary, and will ensure that a
trained RN and other patient care staff are available at all times
to provide excellent patient care. The Director of Clinical
Services will take administrative call per a rotating
schedule.ACCOUNTABLITY:Reports directly to the Executive
