Shift: 8:00 AM - 4:00 PM

Experience:

--- 23 months of role experience is required.

--- Travel experience is preferred.

Requirements:

--- COVID vaccination required for onboarding. Declinations not accepted.

--- Flu vaccination required for onboarding. Declinations not accepted.

Pre-employment modules may be required for this role. Please upload any certifications or health documents you have to your profile to expedite your on-boarding process.

Additional Information:

BLS must be AHA.

Minimum 24 months of Home Health experience is required.

No candidates with an AZ home address on their drivers license will be considered - please include a copy in submission

No RTO

CPI offered onsite

Banner cannot accommodate or guarantee travel pairs or trios.

Shift Notes: 8:00AM - 5:00PM/40 hours scheduled/32 hours guaranteed/Banner is allowed to cancel up to 1 shift per week/Must be willing to float to units where experienced

Weekend Requirements: Weekend schedule to be determined by facility

On Call Requirements: On call schedule to be determined by facility

Maximum days off: 7 days
