Job Description:

Seeking an LPN/LVN to provide skilled bedside nursing care for terminally ill patients according to their care plan, under RN supervision.
Evaluate patient/family needs, assist in developing and implementing interdisciplinary care plans.
Administer treatments and medications, monitor patient reactions, and report significant changes to RNs or physicians.
Maintain a safe, clean environment and accurate documentation of care provided.
Support patient/family education on illness, care, and discharge preparations.
Participate in on-call rotations and flexible scheduling, including weekends or shifts.
Require valid state license, graduation from an approved nursing program, and at least one year of hospital adult medical-surgical experience.
Must have reliable transportation, physical ability for patient assistance, and comply with physical demands.
