We strongly live our value of a work-life balance. Responsibilities Comply with all state-mandated Physical Therapy Practice Act and other legal mandates. Evaluate client for therapy needs and provide written orders/treatment plan for services. Ensure plan of care is consistent with client assessment and needs. Instruct other personnel and/or family members and caregivers on intervention goals. Ensure proper visit documentation completion. Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider. Participate in discharge planning Provide in-service education for agency personnel, as needed. Requirements Current unrestricted Physical Therapist license in state for which care is being provided One (1) year experience as physical therapist, preferably in an in home setting CPR Certification, required Two-step negative TB skin test or chest x-ray, required.
